Driver’s License, Motor Vehicle Fraud Reporting

Lost your driver’s license? Can’t find your registration card? Do you suspect someone else may be using your driver licensing or motor vehicle credentials? PennDOT’s Office of Risk Management wants to hear from you.

Form ORM-1DL – Fraudulent Misuse of ID/Driver’s License Credentials and Form ORM-1VR – Fraudulent Misuse of Motor Vehicle Credentials are provided so individuals can report these events. Both forms are on PennDOT’s website, http://www.dmv.pa.gov. On the Information Centers menu in the upper right corner of the home page, click the Identity and Security link. On the Identity/Security page, click “Report Fraud.” The forms are listed on the right side of the page.

In cases where driver licensing credentials are misused, you must attach copies of your driver’s license (or a current photograph), Social Security card and birth certificate, along with any other documents, such as a police report, that might help PennDOT investigate your case.

Send all information to PennDOT Office of Risk Management, PO Box 69005, Harrisburg, PA 17106-9005.

NOTE: The Office of Risk Management does not handle complaints in regard to citations. To resolve these complaints, contact the court where the citation was filed.
