Motor vehicle agents under contract with the Pennsylvania Department of Transportation (PennDOT), as well as ordinary citizens, have access to most motor vehicle and driver licensing forms online. Fillable versions are available through the “Forms and Publications” tab in the upper right corner of PennDOT’s home page, http://www.dmv.pa.gov/Pages/default.aspx
Agents should always print the most current versions of forms directly from the Web site, rather than keeping stacks of forms in inventory. The exceptions are the multi-part controlled forms – the MV-1, MV-4ST, and the MV-120 – which most be ordered from PennDOT in quantity.
Although these forms are not modified or updated all that frequently, it is possible that PennDOT could reject paperwork that includes an older version of a form. A rejection must be due to information, such as fuel type, or a fee, such as the $5 county fee, that is missing on the old forms.
If you have old forms on hand, know that you could risk rejection of your paperwork if you decide to continue using them.
