The Pennsylvania Department of Transportation (PennDOT) published a new driver licensing form: Form DL-901. The new form combines four forms that are used for learners’ permits, photo ID cards and non-commercial drivers’ licenses:
- DL-31 – Application to add/extend/replace/change/correct non-commercial learner’s permit
- DL-54B – Application for change/correction/replacement or renewal of a Pennsylvania photo ID card
- DL-80 – Application for change/correction/replacement of a non-commercial driver’s license
- DL-143 – Non-commercial driver’s license renewal application
The DL-901 is available on PennDOT’s Web site at http://www.dmv.state.pa.us/pdotforms/dl_forms/DL-901.pdf.
The Bureau of Driver Licensing will continue to accept the four prior DL forms until the end of 2012.
Agents should continue to use their supplies of the DL-31, DL-54B, DL-80 and DL-143 forms they have on hand.
